  2. SSN
  3. HS or College Transcripts (MUST SHOW COMPLETION) OR Wonderlic OR GED OR Attestation form with statement. NO DIPLOMAS!!! (Required no later than class 3)
  4. Immunizations or Declination

Minimum age: You must be 18, but those with a high school diploma can apply at 17 (no younger).

Credentials on completion: “Certificate of Completion” and Basic Life Support (BLS) Certification – Certified through the American Heart Association.
